From UT Health Austin: In anticipation of the expanded approval of the emergency use authorization of the COVID-19 Pfizer vaccine (2-dose series) for minors 12 to 15 years of age, the university is now accepting pre-registrations for this age group. The university will offer appointments to individuals who pre-register when the emergency use authorization is approved by the US Food and Drug Administration (FDA), which could be as early as the week of May 10th.

If you are the parent or guardian of a child age 12 or older and wish for them to get vaccinated through the university (in the Health Transformation Building, 1601 Trinity St Bldg. A, Austin, TX 78712), please submit the COVID-19 Vaccine Form with their information. A parent or guardian must also complete the Minor Consent Form (also available in Spanish) for individuals between the ages of 12 and 17. The completed and signed consent form is required at the time of the vaccination appointment. Forms will be provided on-site.
